LIABILITY WAIVER, RELEASE, AND ASSUMPTION OF RISK

Please read this document carefully before signing. This is a legally binding agreement.

1. PARTICIPANT INFORMATION

This waiver applies to the minor child(ren) named below who are participating in activities provided by Home Grown Joy LLC, including but not limited to children’s camps, birthday parties, and related events.

2. ACKNOWLEDGMENT OF RISK

I, the undersigned parent or legal guardian, understand that participation in activities at Home Grown Joy LLC involves inherent risks, including but not limited to:

• Interaction with farm and domestic animals (including goats, pigs, poultry, dogs, and other animals)

• Animal behavior that may be unpredictable, including biting, scratching, kicking, or accidental injury

• Outdoor activities on uneven or natural terrain

• Slips, trips, and falls

• Exposure to weather conditions

• Pond and water-related activities, including fishing, exploring near water, paddle boat rides, and optional swimming

• Inflatable attractions, bounce houses, water slides, or similar equipment (when offered)

• Campfires, tools, or equipment used for educational or recreational purposes

I understand that these risks may result in injury, illness, property damage, or, in rare cases, serious harm.

3. VOLUNTARY PARTICIPATION

I acknowledge that my child’s participation in all activities is voluntary, and I knowingly and freely assume all risks associated with participation, whether known or unknown, foreseeable or unforeseeable.

4. WATER ACTIVITIES & LIFE JACKETS

I understand that water-related activities are optional and dependent on my child’s comfort level.

I acknowledge that:

• Life jackets are provided by Home Grown Joy LLC

• Life jackets are required for any child entering the water or participating in paddle boat activities

• My child will not be permitted to participate in water activities without proper safety equipment and supervision

5. RELEASE OF LIABILITY

To the fullest extent permitted by Wisconsin law, I hereby release, waive, discharge, and hold harmless Home Grown Joy LLC, its owners, operators, employees, volunteers, and representatives from any and all claims, demands, actions, or causes of action arising out of or related to my child’s participation in activities, including those caused by negligence, except where prohibited by law.

6. INDEMNIFICATION

I agree to indemnify and defend Home Grown Joy LLC against any claims, damages, losses, or expenses (including legal fees) arising from my child’s participation or my failure to comply with safety rules or instructions.

7. MEDICAL AUTHORIZATION

In the event of an emergency, I authorize Home Grown Joy LLC to seek emergency medical treatment for my child if I cannot be reached. I understand that I am financially responsible for any medical care provided.

8. PHOTO & MEDIA RELEASE

I grant permission for Home Grown Joy LLC to photograph or record my child during activities and to use such images or recordings for promotional, educational, or marketing purposes, including on websites and social media, without compensation.

9. ELECTRONIC SIGNATURE

I understand that typing my name below constitutes a legal electronic signature and is the equivalent of a handwritten signature.

10. GOVERNING LAW

This agreement shall be governed by and interpreted in accordance with the laws of the State of Wisconsin.

PARENT / LEGAL GUARDIAN ACKNOWLEDGMENT

By signing below, I confirm that:

• I am the parent or legal guardian of the minor child listed

• I have read and understand this waiver in its entirety

• I voluntarily agree to all terms stated above
